This is heading in a better direction, but here's my issue with this:

if the user presses Cancel then the action in the dialog should not be
taken. That was consistent with what I had before, but is inconsistent
with what you propose here (as the checkbox will stay in effect).

I'd much rather see something like this

User enters name, clicks "OK"
User doesn't enter name, clicks "OK" - this keeps the default
User doesn't enter name, clicks "Ask me next time"
