[PATCH] user-manual (with attachments this time)

I have messed around with asciidoc and the layout. There is now a banner 
that has been put in placed using a workaround by commenting out several 
of the important directives of asciidoc and by brute-forcing the 
software to co-operate. The table of contents has been moved lower down 
in the text after discovering a little-known instruction. There are many 
changes, forming part of a stylistic revision of the text that I started 
while invigilating an examination today. Please let me know when I can 
start updating the graphics. I see most of the screenshots will need 
replacement.
