Subsurface translations move to transifex

It seems a few people haven't set up accounts at Transifex, yet...

Swiss-German, Bulgarian, Norwegian, Portuguese and Slovak appear to be
missing.

I updated the translation texts on the site and would love for people to
take a look so we can get the workflow figured out.
