On Mac OS there “File” menu very often contains an “Open Recent” item that opens a sub-menu listing recent files (and an option to clear that list). So I second the placing there.
A bit of googling produced
https://developer.apple.com/library/mac/documentation/cocoa/reference/applicationkit/classes/NSDocumentController_Class/Reference/Reference.html
They have a class that handles opening new or existing documents and maintains a list of recent documents. I have no idea if that is interfaced with Qt at all. But another thing it does is that there is a system wide “Recent Items” list that separately contains programs and documents and is located in the Apple menu.
Best
Robert
